Welcome to EliteCare

We make it easy to become a patient. Most consultations are scheduled within 24 hours of the initial call, and walk-ins are welcome during office hours. The information below will help you prepare for your first visit so we can spend our time together focused on your care, not paperwork.

If you are being referred by another provider, let us know when you call and we will coordinate directly with their office to get any necessary records in advance.

What to Bring to Your First Appointment

  • A government-issued photo ID
  • Your insurance card
  • A valid credit or debit card to keep on file
  • A referral from your physician, if your insurance requires one
  • Any relevant imaging (CT scans, ultrasounds) or prior surgical records, if available
  • Completed new patient forms (see below)

If you do not have imaging or records, that is fine. We can order what we need after your initial evaluation.

To save time at your first appointment, please download, print, and complete the forms below before you arrive. If you are unable to print them, that is fine, you can also complete them at the office. Please plan to arrive 15 minutes early if you are filling forms out in person.

Patient Registration

Demographic information, contact details, emergency contact, primary care physician, and pharmacy information.

Insurance Information

Primary and secondary insurance details. If you have two insurance plans, complete the additional insurance section.

Authorization to Release Information

A HIPAA authorization form covering how you would like us to communicate with you (mail, phone, fax) and whether we may leave detailed messages.

Patient Financial Responsibility Consent

Our financial policy, including credit card on file authorization, payment terms, insurance responsibilities, and cancellation policy. Please review carefully and sign to acknowledge.

What to Expect at Your First Visit

Your first appointment is about understanding your situation. Plan for the visit to last 30 to 45 minutes.

  • Check-in. When you arrive, our front desk team will scan your ID and insurance card, collect your completed forms, and verify your information.
  • Initial evaluation. You will meet Dr. Zahiri, who will review your medical history, examine you, and look at any imaging you have brought. If imaging is needed, we will order it.
  • Discussion of options. Dr. Zahiri will walk you through your diagnosis, available treatment options, and what he recommends. You will have time to ask questions, and we encourage bringing a family member or friend who can help you take in the information.
  • Next steps. If surgery is recommended, we will discuss timing, hospital logistics, insurance coverage, and the recovery pathway. Our team will follow up with scheduling, pre-operative instructions, and benefits verification.
  • Pricing transparency. For self-pay patients, we provide a written estimate at the time of consultation. For insured patients, our team will verify your benefits and provide an estimate of your out-of-pocket cost before any procedure is scheduled.

Telehealth Consultations

If you prefer to meet remotely or are located outside the immediate area, virtual consultations are available. Telehealth visits are conducted through a secure video platform and follow the same general structure as an in-person visit. Bringing Someone With You

Many patients bring a family member or friend to their first consultation, and we welcome that. Having someone with you can help when discussing surgical options and planning next steps. If you are bringing a caregiver who will be helping with your recovery, we recommend they attend the consultation with you so they can hear the same information first-hand.

Most patients are seen within 24 hours. Same-day appointments are available for urgent cases, and we welcome walk-ins during office hours.

A referral is not required for most appointments, but if your insurance plan requires one, please bring it to your first visit. Some plans require referrals for specialist visits, and we can help you confirm whether yours does.

Yes. If you are unable to print and complete the forms at home, please plan to arrive 15 minutes early so you have time to fill them out before your appointment.

We see uninsured patients on a self-pay basis. New uninsured patients pay a $350 consultation fee at the initial visit. Additional visits may incur charges depending on the care required, and routine post-operative visits included in the surgery price are not billed separately. No. There is no fasting or special preparation required for a consultation. Eat normally, take your usual medications, and come as you are.

If possible, we recommend arranging childcare so you can focus on the consultation. If that is not possible, you are welcome to bring children, though our office is not specifically equipped with a play area.

We accept Visa, MasterCard, American Express, Discover, debit cards, HSA/FSA cards, cash, and certified checks. A 2.99% processing fee applies to credit card payments. Cash and check payments are not subject to additional fees.
